Clinical Pharmacist
JOB TITLE: Clinical Pharmacist
DESCRIPTION: The fulltime Clinical Pharmacist will provide services primarily in the Primary Care Medical Home Port outpatient clinics providing care in support of Naval Hospital Oak Harbor. The Clinical Pharmacist expected to work 40 hours per week; no overtime. A typical work week is Monday through Friday from 7:00 a.m. - 5:30 p.m.
Qualifications:
License & Certification Requirements: Possess and maintain a current, valid, unrestricted license to practice Pharmacy in any one of the fifty states, the District of Columbia, the Commonwealth of Puerto Rico, Guam or the U.S. Virgin Islands.
Education Requirements: A Baccalaureate Degree in Pharmacy from a College of Pharmacy accredited by the American Council for Pharmacy Education (ACPE); or Possess a Doctorate degree in Pharmacy (PharmD) from a College of Pharmacy accredited by the American Council for Pharmacy Education (ACPE) or Possess a graduate certificate within the preceding year from an accredited pharmacy residency program
Experience Requirements: Possess a minimum of 12 months of experience within the preceding 18 months as a Clinical Pharmacist performing direct patient care clinical activities in an inpatient, outpatient, or ambulatory care pharmacy setting or have held clinical privileges as a Clinical Pharmacist within the preceding 36 months in a military treatment facility.
